A UK (Scottish) SME is developing a project proposal for the In Care Heart Pre-Commercial Procurement (PCP) call. They have developed a health data exchange platform to support the virtual management of patients with chronic conditions, e.g. Heart Failure. (TRL 8). The company seeks implementation and technology partners to support the integration of the PCP project. The company is particularly interested in companies from the relevant regions in Sweden, Italy, Portugal, Turkey and Greece.
The UK (Scottish) company has developed a proprietary data interoperability platform which allows the integration of existing clinical data with new sources of patient information acquired from digital services, medical devices and wearables to support the management of long-term conditions. It provides consent, security, authentication, curation, storage and integration with systems of records. On top of the platform, digital services have been developed to address specific conditions providing; a patient web app to capture Patient Reported Outcome and Experience Measures (PROMS, PREMS), provide messaging and rehabilitation support; a clinical portal to allow interpretation of the data, and improved management and care planning. In addition AI models are applied to the datasets to predict events and provide proactive care planning. The platform has been used for the digital management of diagnosis (Heart Failure) and ongoing monitoring COPD (Chronic Obstructive Pulmonary Disease).
The INCAREHEART PCP project brings together five procurers who want to radically improve integrated care for patients with Chronic Heart Failure (CHF). They are looking for an innovative integrated care solution that overcome fragmented health and care service provision. They will jointly procure Research and Development services to shape an ICT-enabled integrated care solution supporting the implementation of a comprehensive multidisciplinary and cross-organisational care and support model for people living with CHF across a fully integrated patient pathway. The company seeks implementation and technology partners to support the integration of this PCP project.
Open market consultation events have already taken place in anticipation of the call for tenders which is expected to be from November 2021 to April 2022. Deadline for expressions of interest for the Scottish company would be end of October 2021.
- Type of partner sought: Within the framework of the INCAREHEART project, the procurement aims to procure R&D services in the form of a solution that provides innovative ICT-enabled integrated care solutions to advance multidisciplinary health and care for patients with chronic heart failure (CHF). The procurement is intended to address the unmet needs of the procurers in relation to various aspects (building blocks) of continuous management and seamless transitions of care for CHF patients:
(a) early diagnosis and anticipatory care;
(b) care and follow-up;
(c) living with heart failure.
Further information regarding technical specification is expected to be given at procurement stage.
- Specific area of activity of the partner: The company is seeking to identify industry collaborative partners to support the integration of the project with one or more of the regional sponsors health systems in Sweden, Italy, Portugal, Turkey and Greece.
This would include potential in-country management of clinical trials and integration of the solution into the respective national/ regional health systems. These partner should have a working knowledge of implementing digital technologies into the regional health systems, preferably with experience of managing European PCP or R&D projects.
In addition, the company would like to speak to technology partners for some elements of the PCP project. This includes:
Cardiac rehabilitation / treatment adherence Devices or sensors which could be applied to CHF.

Evaluation scheme: R&D service contracts will be awarded in parallel to different R&D providers in a phased approach. This will allow alternative, competing solutions to be compared. Each selected supplier will receive a framework contract covering three R&D phases.
